Large Drive Unit error message (lasterr)

I can't solve an error message, PWMSTUCK
I have installed firmware 4.56.R-sine, and parameter file params_panzer_teslaLDU_new_v1.json. included is a copy of the web console.

I have checked continuity from the PCB board connector to the the encoder connector, and well as verified that 5 volts is present on the correct pin. Swapping around the A-B encoder signals has no effect. It is difficult tho determined the shape of the encoder pulses as the motor runs ver rough. Adjusting the pwmfrq parameter to a higher value does seem to allow for a smoother motor, but the jerkiness only increase in frequency.
I am testing the system using a 60 volt source with no more than 5 amps available, just to limit the possible smoke. My test setup is to apply 60 volts to the DC terminals and then power the 12 volts, the IGN, Start, and Forward and all connected to the 12V. No contactors are present at this time.

PWMSTUCK is just a warning and it may even be a false positive. I haven't gotten much feedback on this so it might trigger when there is no problem.
I would expected all pins to read as 0 in active high mode but most of yours read as 1 except 1 (110 111). Maybe you should check if they produce a proper waveform after the buffer IC and also that they are all 0 at powerup.
