Gen3 Prius- PWM present but motor not turning

Problem: My motor won't turn, even though the PWM signal is present. What am I missing?
This is my first time using Open Inverter.
I Inherited a have a Prius Gen 3 inverter with Damien's V1C board (denoted “Block III”). I was told it was working, but I couldn't get it to spin a motor, so I decided to order another board using the newer “Block 4” schematics. I ordered from JLC PCB.
I am trying to run an AC induction motor with an encoder. I'm using the latest (v5.35.R) Sine firmware.
As far as I can tell, the board is running well and producing a PWM signal for the gate drivers.
I have 3 incandescent light bulbs wired in a Delta configuration to the 3 phases of MG2.
When I set fslipspnt = 1 Hz, and Ampnom= 50%, manualstart= on, none of the light bulbs illuminate. I have tried various amounts of current, and PWM frequencies. Pin 3, MG2_FORW_IN is connected to +12V.
I have the system set up with a 48-volt battery. I have tried other voltages, lower and higher up to 150 volts.
In the dashboard, OPMODE is set to "run". Everything looks good, but I'm getting little/no current out of MG2.

There is a PWM signal present on the MG2 phases, but I'm just seeing the frequency of the mains voltage (60 Hz in my case):
Is there a problem with my Gen 3 Inverter? I have a second one, and installing the board on that one gives the same result.

Is there a problem with my parameters? or the hardware version? hwrevs is displayed as Rev2. How can I change it?
I have also connected and setup the throttle, but pressing it doesn't do anything apart from changing opmode to "pot pressed"
I can set up the encoder, but I don't think it's needed yet at this stage, correct?
